Creative Media Center Large Format Inkjet Printing Request GSU Students who are currently enrolled and have been accepted as a declared major within the Ernest G. Welch School of Art & Design use this form a single copy of this form for each request for an Epson InkJet Print from the either the Epson 10000CF (43" wide), Epson 7500 (23" wide), or Epson 4000 Professional (13" wide) Printers. Creative Media Center Digital Equipment Temporary Loan Form GSU Students who are currently enrolled and have been accepted as a declared major within the Ernest G. Welch School of Art & Design use this form to temporarily checkout digital audio, video, or photographic equipment from the Creative Media Center during Open Lab times, when accompanied by the required proper proof of identity, enrollment, and local contact information. Request to Reserve a Creative Media Center Computer Cart GSU Graduate Students who are currently enrolled and have been accepted as a declared major within the Ernest G. Welch School of Art & Design, Full or Part Time Art & Design Faculty members, or Art & Design Graduate Teaching Assistants use this form to temporarily checkout cart-based Macintosh computer equipment from the Creative Media Center during Open Lab times when accompanied by the required proper proof of identity, enrollment, and local contact information.
